Word on the web is it was the launch pad wot dunnit ... or at least an executive decision taken three years ago not to build certain safety features into the launch pad to avoid damage to same. And whaddyaknow, when the engines were fired up, they caused predictable damage to the pad, which went on to cause arguably predictable damage to the engines, and a few minutes later ...
