Hiring of a Composer/Musician for your film

I'm about to place a post on a film forum, here in Seattle, for a Composer/Musician. In doing so, I want to if any of you made a contract agreement with the composer you've used, and, if you know where there is a simple sample of such contract. Even if it's a friend, I feel it's better to seal the deal, then to have them come up and say 'I don't want you to use my music.'

What did any of you do, when you contracted a composer/musician? -Even a recording artist (unknown) for your film projects.
