Post Script Supervisor - Lining a Script

My questions are about lining a script:
(1) Does the lining of the script happen before shooting and/or as each scene is being shot?
(2) Who lines the script for the different shots? (e.g. Scene 22A master shot; 22B medium shot of Jane and John; 22C Close-up of John)?
(3) Is it the job of the Script Supervisor to work with the Director and DP to line the script together, or does the Script Supervisor line the script alone?
My understanding is that the lining of the script is a collaboration with the Director/DP and that is how the shot list gets generated and ultimately supervised.

The shot list is done in advance.

The script supervisor lines the script during & immediately after each shot.

The script supervisor does it alone.
