I suspect you're asking about those hits at 2:05.

No fake skin with something underneath. That's a stuntman
really getting hit. To get that kind of muscle ripple the hit
doesn't need to be too hard, but there needs to be full contact.
Notice that Downey never gets hit like that. And it's not Downey
who hits the stuntman.
