I answered the FX question. But I like your answer better. No surprise there.

Personally, I don't think going with the dummy is the way to go. The reason is the reaction of the person's body to the knife attack is going to look too, uh, wooden. You're better off going with reshooting the closeup if you can with the knife from behind. Then when the girl falls forward you can have FX of the torn clothing and blood. Or, just do a closeup with the actresses reaction/struggle and perhaps blood spatter on the ground? Might as well try several things. Just like the torture scene in "Reservoir Dogs" however, they deemed actually showing it to look too fake, so the ear cutting was off screen with sound. So much creepier.
