contract for sponsorship /equipment use

I want to ask my previous boss to borrow some of the equipment from his production studio to finish a film that I have already started shooting last year. I will just offer him credits on my film as sponsor. Is there any basic contract for this type of deal? Am I asking too much? Is that something that people usually do? I only need to borrow camera/mics/tirpods for just a couple of days. Any suggestions? Thanks. - Emanuelle
