In no particular order: Film in an isolated sound stage. Use a top class professional PSM (with top class equipment). Use good experienced actors (who can project their voice and know how to make noise between their dialogue). Dress the set for silent operation. Links: Almost any well budgeted indie film.
None of these conditions are typically met on no or extremely low budget indie films and even with a high budget indie all these conditions cannot always be met. In which case the best way of recording production sound varies according to the conditions.
