How do you properly attach a lavalier?

I'm working on a no-budget feature, and we've hit a wall with laving up talent. I've had some fantastic help in regards to audio, but I still cannot for the life of me prevent the lavalieres from making noise from contact with clothing.

I've tried having a shirt underneath and over the lav, but no luck. I've tried making a triangle with gaff tape and taping the lav to the inside of a shirt that way, and still have had no luck. And I tried wrapping the capsule with mole skin and it just brings about a new kind of contact noise.

Is there something I'm missing? If any audio experts are feeling generous, could you take me through the process of lav'ing up talent? Thanks a ton!
