I googled to find some release forms online (location, talent, etc.) and found a couple sites. Do the links below have well written documents? Any other good ones to use that any of you know of? I understand some of the phrases, but I'm not a lawyer, either. My sister is, maybe I should ask her to proofread one?

Yes, you need them to sign on behalf of the minor.

Get both the parent and the child to sign. The wording of these child actor release usually refers to "We" instead of "I".
