I just became the 2nd AD for a SAG ultra low budget feature and I am trying to find a production report template (excel) that reflects the camera dept. using a HVX200. Does anyone know how to find one (cheap)?

I was the 1st AD on a movie I produced and I was wondering what kind of money does a 2nd AD get under SAG?

The assistant directors aren't SAG members. The guild that covers AD's is the DGA.
