I'm producing a documentary about this particular family, and they have a lot of home movies and pictures that I'm going to use as B-roll. I have consent forms for the interviews and images, but I need a form specifically to handle the rights for the B-roll. Does anybody have a form like that, or know where I could get one?

Unless they say otherwise, maybe the b-roll is covered under the release you had them sign. My suggestion would be to make sure you can use the pics and old film in the first place. Then if you are still worried, maybe you need to make up another release including the b-roll, just to be safe...
It would seem to me that unless the footage was sensitive, the b-roll would be included in the release and their agreement to do the doc. But the one thing you don't want to do is assume anything. Better to be safe than sorry, so write up another release for them to sign.
