Im new to the forums. I tried to search on the internet and on the forums but I could not find a waiver form that I wold need to get signed by a few businesses.

I am filming at a local spa and a restaurant and wanted them to sign a waver form.

The location release I have has liability and/or lack thereof clauses in it. You might just look for various location releases and see if they have what you need included in them.
